Interesting.
If I had to choose between the first and second design as a real quilt to sleep under, I would choose the second design, because too much quilting compresses the wadding and the quilt will not keep us as warm if it is too heavily quilted. When quilts were padded with cotton bolls the quilting held the bolls in place, but with modern battings we don’t need to quilt as closely.

No piecing, no applique, just quilting lines. Use different fabrics for front and back and the result would be reversible … two quilts for the price of one.
